Massachusetts Licensure — The Honest Reality
Massachusetts is NOT a member of the Physical Therapy Compact. Every PT and PTA practicing in Massachusetts needs a full Massachusetts state license regardless of what you hold elsewhere. Massachusetts has enacted Occupational Therapy Compact legislation, but the OT Compact is not yet operational in MA — OTs also need traditional Massachusetts licensure for now. Speech-language pathologists should verify ASLP-IC compact status with the Massachusetts board before relying on compact privilege.
What this means practically:
- Timeline: Plan for 6-12 weeks for endorsement from another state to active Massachusetts license.
- Fees: Initial PT licensure is around $150-$200; OT and SLP fees are in similar ranges. Verify current amounts with the board before paying.

Where the Assignments Are
Massachusetts is dominated by the Greater Boston healthcare market, with one of the highest concentrations of academic medical centers in the country. Major regions:
- Greater Boston — anchored by Mass General Brigham (the combined MGH and Brigham & Women's system), Beth Israel Lahey Health, Tufts Medicine, and Boston Children's Hospital. Highest volume in the state, with extensive specialty and acute-care opportunities. Expect competitive pay packages but high cost of living.
- Worcester (central Massachusetts) — UMass Memorial Health and Saint Vincent Hospital lead the area. Lower cost of living than Boston, with consistent demand across acute care, SNF, and outpatient settings.
- Springfield (western Massachusetts) — Baystate Health is the major system, with strong demand across all settings. Significantly lower cost of living than Eastern MA.
- Cape Cod and the Islands — Cape Cod Healthcare and Martha's Vineyard Hospital. Strong seasonal demand peaking in summer; housing inventory is very tight in peak season.
- North Shore and South Shore (Boston suburbs) — Beth Israel Lahey, South Shore Health, and Mass General Brigham community hospitals. Often easier housing than central Boston with strong commuter rail access.
Massachusetts Tax Reality
Massachusetts has a flat state income tax of 5% on most income, plus a 4% surtax on income above $1 million (the "Millionaires Tax," effective 2023). For most travel therapists, only the 5% rate applies.
Practical considerations:
- No reciprocity agreements with neighboring states. Travelers from CT, NH, NY, RI, and VT file MA non-resident returns on MA-earned income.
- Your home state typically provides a credit for MA taxes paid, preventing double taxation.
- New Hampshire residents have historically had a unique situation around remote work taxation — this matters less for in-person travel assignments but worth noting if you're an NH resident.
- Tax-free housing and M&IE stipends still require legitimate tax home documentation regardless of which state you live in.
The 5% flat rate is moderate compared to high-tax states like California or New Jersey, but takes a meaningful bite from take-home pay compared to no-tax states.
Massachusetts Housing for Travelers
Greater Boston housing is among the most expensive in the country. The housing stipend typically falls short of a private one-bedroom in Cambridge, Brookline, or downtown Boston — most travelers opt for shared housing, North or South Shore commuting, or extended-stay properties farther from the urban core.
Worcester and Springfield are far more affordable. Stipends comfortably cover quality housing with money left over.
Cape Cod is the housing-trap of the state: in-season inventory is scarce, expensive, and often booked months ahead. Off-season housing is plentiful and affordable. Confirm housing before accepting a summer Cape assignment.
Practical note: New England winters are real. Heated parking, snow removal arrangements, and short walks from car to door matter more in MA in February than in TX in February. Factor weather into housing selection.
What to Look for in a Massachusetts Travel Assignment
Massachusetts offers some of the best clinical learning environments in the country alongside some of the most expensive housing. Things to weigh before signing:
- Boston cost-of-living math: A $2,500/week package in Boston buys a very different lifestyle than the same package in Worcester or Springfield. Run the housing math before signing.
- Academic vs. community setting: Mass General Brigham facilities run differently than community hospitals — pace, complexity, expectations all differ. Clarify which you're walking into.
- Full Massachusetts license required: MA is not a PT Compact member. Plan for 6-12 weeks of endorsement processing if you don't already hold an MA license.
- Cape Cod summer caution: Beautiful market, terrible housing logistics in season. Confirm housing first, sign contract second.
- Winter weather realities: Late January through early March can affect commutes. Verify contract flexibility around weather, especially for early-shift assignments.
